Key Responsibilities:
- Conduct in-person comprehensive assessments, including initial, semi-annual, and unscheduled evaluations, while providing continuous medical coverage to participants.
- Collaborate with physicians and the interdisciplinary team (IDT) to create and implement individualized care plans for senior participants.
- Participate in discharge planning efforts from acute or long-term care settings in coordination with IDT and physicians.
- Regularly visit and support participants in nursing homes as needed, fostering a consistent care environment.
- Work collaboratively with community-based PACE physicians and IDT to ensure effective coordination of care.
- Review and analyze outcomes and utilization data, including hospitalizations and specialist use, to enhance care practices and policies as directed by the Medical Director.
- Be part of the rotational night and weekend call team.
Qualifications:
- Graduation from an accredited School of Nursing.
- Certification as a Nurse Practitioner with Prescriptive Authority.
- Current NP State License.
- Minimum of two years’ experience working with the senior population.
- Comprehensive knowledge of chronic care and geriatric best practices.
- Valid State Driver’s License with a clean driving record.
- Proof of recent vehicle insurance.

Salary Range: $182,000 - 254,499, with additional incentives of up to $25,445. Compensation will be adjusted based on experience and qualifications.
